Certification of matura certificate (or apostille if the document was issued by an institution operating in the education system of a State party to the Hague Convention of 5 October 1961 abolishing the requirement of legalization of foreign official documents - Journal of Laws of 2005, No. 112, items 938 and 939).

Document stating that the certificate entitles to take or continue studies in the country of its issue, issued or endorsed by the school or educational institution in which the study took place or by the educational authority of the State in which territory or in which educational system the certificate was issued.

For stay in Poland: confirmation of legalization of stay in Poland: copy of visa or permit for permanent residence/settlement (permanent residence card) or temporary stay permit/temporary residence permit; The temporary residence permit should be accompanied by the decision on issuance of a temporary residence permit for a fixed period of time, or a copy of the document confirming the status granted in Poland (e.g. Pole’s Card, Refugee Card).

For studies conducted in Polish - if required - a copy of the document (with the original for inspection) confirming knowledge of the Polish language, i.e. a certificate of completion of the annual preparatory course to study in Polish in units designated by the minister competent for higher education or certificate of knowledge of Polish language issued by the State Commission for Certification of Knowledge of Polish as a Foreign Language, or Certificate confirming knowledge of Polish language issued by PWSTE in Jarosław.

Certified translation into Polish of a certificate, legalization/apostille (if prepared in a foreign language); and the above document stating that the diploma entitles to take or continue studies prepared by a person entered by the Minister of Justice into the list of sworn translators (list of sworn translators is available on the Ministry of Justice website) - translated documents may also be notarized.

One up-to-date photo, in accordance with the requirements for the issuance of personal identification (this photo, in electronic form, must be included in the system during the electronic registration of the candidate).

Medical certificate of lack of contraindications for higher education (with translation if issued in a language other than Polish).

Medical certificate issued by the occupational medicine doctor for the following majors Nursing, Cosmetology (referral for examination will be received by candidates will be received at the PWSTE Recruitment Office in Jarosław).

Certified copy of passport, photo page and number (original for inspection).

The recruitment fee ( PLN 85) is paid to the account indicated by the university, the foreigner pays the fee to the college cash office or to the account. Read more about RECRUITMENT FEE.

In case if the candidate can not appear personally, there is the possibility of authorizing another person. For this purpose the candidate is required to complete the forms below: POWER OF ATTORNEY (filled out by the legal guardian, eg the parent in case the candidate is under-age; when the candidate is of age, the legal representation document must be notarized) or AUTHORIZATION TO SUBMIT DOCUMENTS BY A THIRS PERSON.
